We found it to be VERY helpful to make up a page on the computer with all of Jack's medical information.  Whenever we met someone new (doctor, nurse, case manager, therapist, social worker, etc.) we just hand them a copy to keep and it saves SO much time, effort & grief of repeating the same dreaded info.  In addition, if it's on your computer then it's easy to update, which we have done often.  Click here if you'd like to see ours (with our personal info removed).
